Appointment Cancellations

We kindly request that you contact our office at least 24 hours in advance if you know you will be unable to keep your scheduled appointment so that we may offer it to another patient in need. A fee of $25.00 may be assessed to your account if you fail to keep an appointment and do not provide notice of needing to cancel. These fees are not paid by insurance and are the patient’s responsibility. This fee will need to be paid before you will be able to be seen by a provider. Missing more than two appointments without providing notice is grounds for discharge from the practice.


Late Policy

If you are more than 15 minutes late for a scheduled appointment, it will be necessary to reschedule your appointment for another day so that we may keep our schedule flowing without interruption to other patients.


Prescription Refills and Prior Authorization

We currently accept many commercial insurance, Medicare, & TennCare Plans but are not in network with all plans.

Payment for noncovered services, deductibles, coinsurances, and co-payments are expected at the time of service. For your convenience, we gladly accept Visa and Mastercard for credit and debit card payments. We also accept checks and cash. Payments can be made through the patient portal as well as in person at the office.

When you need a prescription refilled, you should contact your pharmacy or make the request via our online patient portal. Please do this at least 48 hours in advance to allow your provider time to review your chart prior to making the decision to refill. A prior authorization will be completed if necessary and based on failed therapies. If the medication is still not covered after the initial prior authorization is completed, it will be up to the patient to discuss coverage with the insurance or a change in medication will be offered if deemed medically safe and appropriate.
